ALL APPLICANTS MUST BRING ALL OF THE PROPER INFORMATION
OR THEIR APPLICATION WILL NOT BE TAKEN. WE DO NOT TAKE
APPLICATIONS OVER THE PHONE OR USE INFORMATION FROM PREVIOUS
YEARS. IF APPLICANTS ARE HOMEBOUND AND CANNOT GET OUT,
MEAL DELIVERY FOR THANKSGIVING AND CHRISTMAS ARE AVALIABLE.
We will be taking Christmas applications on the following
dates at the Salvation Army:
7 & 8 October, 9am - 3pm
21 & 22 October, 9am - 3pm
28 & 29 October, 9am - 3pm
5 & 6 November, 9am - 3pm
APPLICANTS MUST BRING THE FOLLOWING INFORMATION:
1. A photo ID.
2. Social Security card
or proof of SS# for every adult household member.
3. Proof of birth dates
of children in the household. Medical cards will
not
be accepted since there is no longer a birth date on them.
Only children
who are living in your household and are in your custody are
eligible.
4. Proof of monthly
income and source of income. (Paycheck stubs, a letter
of verification from K-TAP, Social Security, SSI or proof of
child support
are acceptable.)
5. Proof of rent and
utilities. (Copies of bills, cancelled checks or
payment
receipts.)
6. Sizes of children's
clothing, shoes and toy choices.
YOU MUST HAVE ALL INFORMATION WHEN YOU
APPLY OR YOUR APPLICATION WILL NOT BE TAKEN. YOU MAY
NOT RETURN INFO AT A LATER DATE.
